Internal Resources - Support Center & UKG:
- To report a positive COVID-19 test, use this link: Reporting a Positive COVID-19 Test
- To report COVID-19 symptoms or if you have been identified as a close contact with someone who has tested positive for COVID-19, use this link: Reporting COVID-19 Symptoms and/or Close Contact
- To request COVID-19 Paid Time off for vaccination and/or booster appointment, as well as for recovery from side effects, use this link: Request COVID Vaccine/Recovery Paid Leave. If a team member contracts COVID-19 and needs time off for isolation and recovery, the team member must request paid leave (PTO, Diversity Day, PPT) in UKG or use unpaid time.

External Resources - Government & Insurance Coverage:
- Order FOUR (per household) at-home COVID-19 tests at no charge via COVIDtests.gov.
- Health Insurance Reimbursement for At-Home Tests: BCBS will reimburse up to EIGHT tests every 30 days for each team member on the plan. Team Members will not need to pay up front. Bring the test kit to the pharmacy counter (not the front registers) at a preferred pharmacy and show your member ID card or pharmacy benefit card. - Medicare or Minnesota Health Care Program (Medicaid, MinnesotaCare) plans: Team Members enrolled in Medicare plans have different options available to receive COVID-19 tests at no cost. See the FAQ provided by the Centers for Medicare and Medicaid Services (CMS) to learn more.
