Team members should be self-reporting with a service ticket in the Support Center for the following, immediately:
- Reporting COVID-19 Symptoms and/or Close Contact: If you have a close contact with a COVID positive individual or are experiencing COVID symptoms
- Reporting a Positive COVID-19 Test: If you have tested positive for COVID
Isolation and Quarantine are different based on vaccination status, if you do not have your status updated in UKG, you will be considered un-vaccinated. Please ensure your vaccination status is updated in UKG, instructions HERE
If you have a severe case of COVID and need to be out for more then 10 days, we recommend you see a physician and submit an FMLA Notification Form (attached)
