Team members should be self-reporting with a service ticket in the Support Center for the following, immediately 




Isolation and Quarantine are different based on vaccination status, if you do not have your status updated in UKG, you will be considered un-vaccinated. Please ensure your vaccination status is updated in UKG, instructions HERE


If you have a severe case of COVID and need to be out for more then 10 days, we recommend you see a physician and submit an FMLA Notification Form (attached)
