This article shows teams how to report safety hazards at Centerspace. All hazards should be reported via the Maintenance IQ (MIQ) work order system using the below steps for mobile or desktop. See Types of Hazards to report for more information about what hazards to report. 


Once a hazard work order has been entered, it must be remedied by following our standard work order procedures. The Centerspace Safety Committee will review hazard work order periodically to ensure proper remediation has taken place. For any questions related to this process, please submit a ticket to the Facilities > Risk Management department.


There are three major steps you need to remember when filling out a hazard work order that are different from filling out other work orders. The steps should be completed in this order:

  1. Select the "Hazard" template.
  2. Change the "Category" and "Subcategory" fields.
  3. Fill out the necessary information in the "Description" field.


Below you will find step-by-step instructions for how to complete a hazard work order in MIQ. This article includes:

How to report hazards using MIQ mobile


1. Login to the MIQ app on your mobile device. The inspections screen is your default. To add a work order, first open the menu by tapping on the three lines on the upper left of the screen.


2. The select "Add Work Order" at the bottom of the menu.



3. Enter your "Property" as you normally would when entering a work order.


4. Then, tap on "Details" in the menu tab to enter further information.


5. Immediately scroll down to the bottom and choose a template. In order to choose a template, select the three dots on the right of the "Template" field.



6. A menu will appear and you should select the "Hazard - Hazard Concern & Corrective Action" template. The form will automatically populate the required fields.



7. After selecting the template, scroll up to change the "Category" field. Although the form automatically populates the "Category" field with "Hazard - Biological", there are six types of hazard categories to choose from. Click on the dropdown to change the "Category".


8. Select the most appropriate category from one of the six hazard categories at the bottom of the drop down menu.  


9. Then, proceed to select the appropriate "Subcategory". Click on the dropdown to change the "Subcategory".


10. Select the appropriate "Subcategory" from the drop down menu.


11. Next, scroll up to add the necessary information to the "Description" field. You can enlarge the description field by dragging down the box on the lower right-hand side of the field. (This can be tricky, because you have to be very exact, but it is possible.)


12. You must fill out the eight pieces of information below. You need to add information to all sections ending with a colon (:).


13. Once complete, scroll down to hit "Save" at the lower left side of the Add WO screen.



How to report hazards using MIQ desktop


1. Login into MIQ on your desktop and proceed to the Work Order dashboard.


2. Click "Add New" on the far right hand side of the screen.


3. After you select your property, choose a template under the details section of the work order submission form. In order to choose a template, select the three dots on the right of the "Template" field.


4. A menu will appear and you should select the "Hazard - Hazard Concern & Corrective Action" template. Then hit "OK". The form will automatically populate the required fields. 



5. After selecting the template, immediately change the "Category" field by selecting the most appropriate category from one of the six hazard categories in the drop down menu. Although the form automatically populates the "Category" field with "Hazard - Biological", there are six types of hazard categories to choose from. 


6. Then, proceed to select the appropriate "Subcategory" from the dropdown menu.


7. Next, add the necessary information to the "Description" field. You can enlarge the description field by dragging down the box on the lower right-hand side of the field.

9. Once complete, click on "Save" on the upper right side of the Add Work Order screen.
